How to answer in Japan
★Do not hand it in blank. If you cannot read it, ask right there. ★Point at the line and say これ、なんて書いてありますか — you can find out what it says without being able to read it. This one sentence works at a hospital and at the city office. Once you are used to it, これは? on its own is enough.

Handing it in blank. If you leave it empty, the doctor has nothing to go on, and nobody knows you could not read it.
